            Mr. William E. Burch (F-116 Vandenberg AFB) has been approved as a leave recipient under the Voluntary Leave Transfer Program due to a Staph infection developed one week following back surgery requiring emergency surgery, extended hospitalization, at home IV Infusion therapy and extended recovery time. 

The Voluntary Leave Transfer Program allows civilian employees to transfer annual leave to another civilian who has a medical emergency (self or qualifying family member) and is without availability of their own paid leave.  Anyone wishing to donate annual leave to Mr. Burch  can do so by completing a leave donation form (OPM 630-A) available at the following link http://www.opm.gov/forms/pdf_fill/opm630a.pdf and turning the original form (faxed or e-mail copies cannot be accepted) to the Civilian Personnel Flight (30 FSS/FSMCE, Bldg 11777, Room B-103).  If you have any questions about the program, please contact Marta Mitchell at 606-5847.
